All parents/ carers and external providers must ensure they read and understand the guidelines listed in the Nuwarra Public School Guidelines for Therapy Provision (pdf).

Parents/ carers, please read the Department of Education’s Information for Parents and Carers and complete Parent Request for the Provision of Therapy Services (pdf), then return the form to the school.

4. Completed child protection training- for the current year
5. Proof of identity - 100 points

A new parent request must be filled in by the parent/ carer each year.
